How to get referred

You need a referral from your doctor to become a patient in our Oncology and Malignant Haematology Clinics.

When making a referral, your doctor will include:

  • A letter of referral
  • Clinical notes
  • Tests, X-rays, and any other relevant results

Referral information for physicians

  1. Download and complete the referral form.
  2. Include the following documents: clinical notes, tests, X-rays, and any other relevant results, pathology results with biomarkers, and staging (CT scan results, MRI, bone scan, etc.).
  3. The referral form will be reviewed to ensure that it is complete. If incomplete, it will be returned to your office.
